Minor Waiver Form

In the state of Alabama you are considered a minor until the age of 19. Therefore if you are not 19 or older you must have a parent of legal guardian sign the waiver for you. The waiver can be signed while the parent or guardian is present at the park or if the parent or guardian will not be present to sign they can print the waiver from below and sign it in the presence of a notary public. (notary seal must be on waiver to be valid) No Exceptions!
Note to parents: Parents must send in a copy of their drivers license with waiver form. Once the waiver is signed Boggs & Boulders staff will grant admission to the minor for a period of one year unless notified otherwise by you. Your childs attendance is your sole responsibility.
